Background: Antiresorptive drugs used to treat various diseases, inducing accelerated bone resorption, may result in medication-related osteonecrosis of the jaw (MRONJ). The evolution of this condition is characterized by osteosclerosis and osteolysis processes, with the formation of bone sequestrum without a healing tendency. Case report: We present the case of a 62 year-old patient under bisphosphonate treatment, in whom mandibular osteonecrosis was developed after a dental extraction. Disscusion: The healing capacity of the jaw bone is affected by antiresorptive agents. When present, the periosteal reaction has low intensity. In our case, the osteogenic activity of the periosteum led to the deposition of neoformative bone in a large amount, disposed similarly to the healing processes occurring consecutively in chronic osteomyelitis.
