Introduction. In patients with diabetes, caries prevention is necessary.The aim of the study is to evaluate the effectiveness of local caries prevention measures with the use of fluoride-free products in patients with diabetes mellitus.Materials and methods. 95 patients with type 1 diabetes mellitus aged 18 to 34 years were examined in two groups: main (50 subjects) and comparison (45 subjects). Patients of the main group underwent local caries prophylactic measures according to differentiated approaches, depending on the degree of caries risk, using fluoride-free products containing nano-hydroxyapatite, xylitol and enzyme products. Conventional fluoride-containing measures were performed in the comparison group. After 6 and 12 months, the intensity of dental caries, increase in the intensity and reduction of increase in caries intensity, structural and functional acid resistance of enamel, the rate of its remineralization, and hygienic index OHI-S, were determined. Results and discussion. In the main group, the increase in caries intensity after 6 months was 0.1, after 12 months it reached 0.3 while, in patients of the comparison group – 0.3 and 0.8, respectively; reduction in the increase in caries intensity was 62.5%. After 12 months, in patients of the main group, the structural and functional acid resistance of tooth enamel improved from 5.26±0.27 to 3.14±0.16 (p < 0.05), enamel remineralization accelerated from 5.46± 0.34 days to 3.24±0.24 days (p < 0.05), and the state of oral hygiene improved from 2.10±0.09 to 1.39±0.07 (p < 0.05). In patients of the comparison group, the final studied indicators were significantly worse (p < 0.05). Conclusions. Local caries prevention measures using fluoride-free products in patients with diabetes mellitus have shown a high effectiveness which, in terms of reducing the increase in the intensity of dental caries, was 62.5%.
