The insulary myocutaneous flap of the great pectoral muscle (MCFP) offers several advantages, such as: localization in the proximity of head and neck, simple collecting method, possible application in cases in which other reconstructive surgical technique fails. The clinical steps of the plastia of post-surgery defects in the oromaxillofacial area, by means of the musculo-cutaneous flap of the pectoral muscle, are described with a clinical case. Due to its good vascularization, this type of flap can survive in less favourable conditions, for example when transferred to an infected area or to an inflammed tissue, as well as in cases of contamination with saliva. Reconstruction of the defect in the donor zone is relatively simple, due to the tissues still available after flap collecting.
